Microsoft Customer Agreement Acceptance

We are required by Microsoft and our provider to get the Microsoft Customer Agreement Signed and agreed to.  Luckily Microsoft will accept us completing the form on your behalf and no physical signature is required.  We are only required to enter the same information we are requesting below.
